Jermaine Elliott Levy was born February 9, 1973 at the Mary Immaculate Hospital, Jamaica, NY to his parents John and Joyce Levy. He attended the local schools in Duval County and graduated from William M. Raines High School.

Jermaine departed this life March 7, 2012 and was preceded in death by brother, Charles Edward Levy; father, John Levy III; maternal grandfather, Bishop Ellison James, Bronx, NY.

He leaves to cherish his memory mother, Joyce Ellen James Levy; brother, Derrick Levy; maternal grandmother, Daisy James, Bronx, NY; special friend, Stephanie Foster; step children, Paul A. Foster and Wynanna K. Foster and a host of aunts, uncles and cousins. The funeral service for Jermaine Elliott Levy will be held 11AM, Saturday at the Greater New Jerusalem Baptist Church 207 West 6th Street.
